Invisalign Teen Treatment Contest! Entries will be taken by submitting your name, address, age, email and phone number to the sponsoring site.
PRIZES & APPROXIMATE RETAIL VALUE: The winner will receive an American Express Gift Card valued at $500. Illegible, incomplete, mutilated postage due or mechanically reproduced entry forms or entry forms that have been tampered with will be disqualified.

PLEASE post the expiration date in the beginning of a web-page
